General Liability Claim Reporting Instructions

“A guest incident just occurred on my property. What should I do?”
● Call Becky Cummings at Connell Insurance – her contact information is provided below. She will take all information needed to report the claim to the carrier. She may ask you to complete the General Liability Notice of Occurrence form found below and submit it to us.
● Does someone need medical attention? They should seek care if needed, but the adjuster assigned to your claim is the only one who can give authorization for your carrier. Advise the claimant that you will report to the carrier and an adjuster should be in contact with them ASAP, usually within 24-36 hours.
● Is there a witness? Go ahead and obtain a statement from them in writing.
● If possible, take photos of the area where the incident occurred.
● Is it after normal business hours? If it’s an emergency situation, you can call 800-356-8140 to speak to our 24-hour service center. They can help you get things started until you can speak to Becky.
● Relax. This is what we do.

“What happens next?”
